Ted Lewis. It was out book of the month in March 1999, and there's some discussion about it in the archives:

I searched the archives on "get carter" and turned up a fair number of hits. The book (aka JACK'S RETURN HOME) has a number of fans on the list, though I couldn't get into it myself.
