But the Cook-Miller index sounds true, it's probably the same Michael Cook who edited "Detective, Mystery and Espionage Magazines" I mentioned. Amazon knows the book, but can't deliver it. And there you could have the whole title. I picked it out of some web page, but can't just now remember the address.
