I've always thought both Johnny and Al died too. The voice on the phone
says,"That's as far as we go , bud," which is important too. After all,
Tony had not done what he was supposed to do, which was to get the girl
[Eve Cressy] out of the hotel. "Al had a hunch you'd run him [the guy]
out," says the voice. Did his late brother cover for him (last good
thing), or did he not cover for him, but it didn't matter because they got
Johnny? Did Al know the guy was already there? Teasing details.

Once nice connection, maybe intentional: Johnny says, in response to Tony's
help, "They'll love that...They'll send you violets." At the end, he's
left with Eve Cressy, asleep in the lobby; she has "deep violet eyes."
