As I have mentioned before, for my money the best ending I can recall is from PICK-UP by Charles Willeford. This ending forces you to reevaluate every scene, every circumstance involved from beginning to end. It is so effective that is spite of this being one of the most nihilistic reads ever one is almost coerced into an instant reread thanks to this ending. Yes, a "twist, surprise" ending but far from a gimmicky one.
