Seems the original English manuscript was lost and the finished book was cobbled together from two partial manuscripts. Since we've got John here, I was wondering if you could confirm my impression (or has a full copy come to light in the intervening decade?) and tell us how that worked. For instance, did new material have to be inserted to graft the parts together?

Short answer is the full manuscript did indeed come to light. As of right now I'm not sure what the deal is re. US publication. Probably best to get a UK copy from Amazon.
